29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used: (a) 2nd floor, North west room, on wall to left of door: On or about 21 June 2023, and at times prior, the employer exposed employees to fire and electrical shock hazards in that a Southwire SJTW Plastic Guard Sting Light was spliced together and covered with electrical tape after being completely severed. (b) 1st floor, North west room, on floor next to doorway on left: On or about 21 June 2023, and at times prior, the employer exposed employees to fire and electrical shock hazards in that a Southwire SJTW Plastic Guard Sting Light was severed and the end repaired incorrectly with electrical tape.
