(b) 1st floor, North west room, on floor next to doorway on left: On or about 21 June 2023, and at times prior, the employer exposed employees to fire and electrical shock hazards in that a Southwire SJTW Plastic Guard Sting Light was severed and the end repaired incorrectly with electrical tape.
