29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  On or about August 11, 2023, the employer did not ensure that Sanitation employees implemented energy control application steps prior to allowing them to engage in servicing and maintenance activities on while performing the "Dry Cleaning" in the "Secondary" production rooms exposing them to pinch/nip point hazards and amputation injuries.   As a result, the following steps were not implemented in order involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about July 10, 2023, the employer did not provide one or more methods of machine guarding to protect employees working along the CS-1 line. Employees were exposed to caught-in nip point hazards due to the gaps present in the slatted conveyor belt, which could result in injuries such as abrasions, lacerations, crushing or amputation injuries.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees.   On or about June 22, 2023, the employer had two instances in the main forklift battery charging room which electrical equipment was not free from recognized hazards exposing employees to electrical shock hazards. The following instances were observed:  (a) On or about June 22, 2023, there was an electrical box on the wall in the main forklift battery charging room that was damaged. The box was bent in, and the cover was not secured, exposing employees to electrical shock hazards.  (b) On or about June 22, 2023, there was battery charging with a damaged power connector/plug in the main forklift battery charging room, exposing employees to electrical shock hazards. This battery sat directly below a sign that said, "BATTERY 21."  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303 (b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.  On or about June 22, 2023, there were multiple locations observed to have unused openings in electrical boxes that were not effectively closed exposing employees to electrical shock injuries and electrocution.  The following instances were observed: (a) A metal wall mounted square box coming off 480V Panel B4B labeled "UPS-DP CKTS 13-15-17, 19-21-23, 25-27-29" was damaged in the forklift charging room with unused openings not effectively closed.  (b) A metal wall mounted square box coming off 480V Panel B4B labeled "UPS-DP CKTS 26-28-30" in the forklift charging room with unused openings not effectively closed.  (c) A metal wall mounted square box labeled "PANEL VRP 1 CIRCUIT 14" observed near a satellite forklift charging area with unused openings not effectively closed.  (d) A metal wall mounted square box labeled "PANEL VRP 1 CIRCUIT 16" observed near a satellite forklift charging area with unused openings not effectively closed.  (e) A metal wall mounted square box labeled "PANEL VRP 1 CIRCUIT 18" observed near a satellite forklift charging area with unused openings not effectively closed.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  On or about September 7, 2023, there was insufficient working space for 480V switches in the ammonia refrigeration equipment room. Obstructed access to electrical equipment can prevent personnel from shutting down the source of power in the event of an emergency exposing employees to increases risk of injuries that could be fatal.    The following instances were observed: (a)  EF-1 480V Switch blocked by "AMMONIA WASTE" drum/barrel. (b) EF-2 480V Switch blocked by stacked buckets and stacked bins. (c) EF-3 480V Switch blocked by buckets and what appeared to be a spare pump/motor. (d) Allen-Bradley PowerFlex West Fan Control module blocked by barrel with quick dry mats stacked on top. (e) Allen-Bradley PowerFlex West Fan Control module blocked by barrel with quick dry mats stacked on top. (f) Allen-Bradley PowerFlex West Fan Control module blocked by "AMMONIA WASTE" drum/barrel.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
