29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about May 26, 2023, in the Green Plant, where an employee was exposed to struck-by hazards while operating a 5 Wire Drawing machine with an unguarded Shaker pan plate. b) On or about June 22, 2023, in the Green Plant, where an employee was exposed to caught-by hazards while operating a Promostar T12 with sensors not functioning.

On the outside of the Green Plant fabrication shop, near the rear of the building, where employees were exposed to electrical hazards while operating a Cincinnati Press Brake with a pedal wrapped in black electrical tape where a flexible cord was not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.
