29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about June 22, 2023 a wooden platform 8' Long by 7' 4" wide was added to the forks of a Hoist Lift Truck-Model F160, Serial #27290 by the employer without manufacturer's prior written approval. The equipment was used to lift an employee up to clean out gutters at Gordy's Lake Front Marine, 320 Lake Street, Fontana, WI 53125.
