29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about June 21, 2023, at 1026 Scramble Drive, Daytona Beach, Florida: the employer exposed an employee to the hazard of being struck in the head by falling objects, in that he was not wearing a hard hat, while stacking plywood for decking and picking-up debris around the building.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  On or about June 21, 2023, at 1026 Scramble Drive, Daytona Beach, Florida: the employer exposed employees to eye injuries by flying debris, in that 2 employees using a pneumatic nail gun and a circular saw to install roof sheathing were not wearing safety glasses.

On or about June 21, 2023, at 1026 Scramble Drive, Daytona Beach, Florida: the employer exposed employees to a 9 feet-4 inches fall hazard, in that 2 employees were sheathing the roof on a 6:12 pitched roof without a fall protection system.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.  On or about June 21, 2023, at 1026 Scramble Drive, Daytona Beach, Florida: the employer exposed employees to a 9 feet-4 inches fall hazard, in that employees doing roofing work on a 6:12 pitch roof were using a Louisville aluminum extension ladder that did not extend 3-ft above the landing surface.
