29 CFR 1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches, plus or minus 3 inches, above the walking-working surface: Employees engaged in animal feed production activities at the Custom Feed Corporation factory located at 2100 North 13th St, Norfolk, Nebraska were exposed to fall hazards in that the elevator pit did not have an adequate guardrail. The elevator pit is 12 feet deep and is guarded by a loose hanging chain that is not fastened to the support posts. The top chain was measured at 49 inches above the walking working surface.
