29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about June 21, 2023, employees working inside of a trench greater than four feet in depth were not provided with safe access, exposing them slip, trip and fall hazards while entering and exiting the trench. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
