Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  47 Broad Street, Auburn - The employer did not ensure eight employees conducting roofing work 20 feet from ground level, on a low sloped roof, had fall protection or a monitoring system in place.

29 CFR  1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   47 Broad Street, Auburn - The employer had extended and positioned a 24-foot aluminum extension ladder at the maximum length to access the roof of a building.  The ladder did not extend 3 feet above the landing surface and the employer did not provide a grasping device to assist the employees in mounting and dismounting the ladder.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  47 Broad Street, Auburn - The employer did not ensure that employees received training on fall hazards and the procedures to be followed to minimize the hazards prior to working on a roof approximately 20 feet in height from the ground.  MPD Master Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1604882 citation number 1 item number 3 and was affirmed as a final order on November 3, 2022, with respect to a workplace located at 28 James Street, Auburn, Maine 04210.
