29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   On or about June 23, 2023, employees were exposed to hazards in the event of hydraulic component failure when working from an elevated platform supported from a Genie Forklift Model GTH-844 and the employer had not ensured the forklift operator had been trained and evaluated in topics such as, but not limited to the requirements of the standard.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  On or about June 23, 2023, an employee was exposed to eye injuries while operating a pneumatic nailer and the employer had not ensured the use of eye protection.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  On or about June 23, 2023, employees were exposed to fall hazards above ten feet when working from a platform elevated by a Genie Forklift Model GTH-844 and the employer had not ensured the guardrail system was installed on all open sides of the platform.
