29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  154 Seymour Ave., Springfield , MA: On or about June 23, 2023, employees were observed working on a residential structure engaged in framing activities with unprotected sides and edges greater than 11 feet above the next lower level  and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.  Jesse Castellano DBA Castellano Home Improvement was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.501 (b) 13, which was contained in OSHA inspection number 1412271, citation number 1, item number 1  and was affirmed as a final order on 9/27/2019, with respect to a workplace located at 43  Preston St., Holyoke, MA.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not  extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  154 Seymour Ave., Springfield, MA: On or about June23, 2023, two employees were observed using a multi position aluminum ladder that did not extend the required 3 feet past the upper landing surface at this residential structure, exposing the employees to fall from height hazards.  Jesse Castellano DBA Castellano Home Improvement was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.501 (b) 13, which was contained in OSHA inspection number 1412271, citation number 1, item number 2  and was affirmed as a final order on 9/27/2019, with respect to a workplace located at 43  Preston St., Holyoke, MA.
