On or about June 22, 2023, the employer did not conduct a hazard assessment in the shipping department to determine if hazards were present or likely to be present. This condition exposed employees to struck-by hazards.

29 CFR 1910.178(m)(6): The employer did not ensure that safe distance was maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car. Trucks were used for opening and closing doors and is not permitted. On or about June 22, 2023, employees working in the shipping area were exposed to struck by hazards when a forklift was being used to close the door on a storage container.
