29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: In the hallway of the back storeroom at 2267 Northampton St, Holyoke, MA: On June 26, 2023, and at times prior, relocatable power taps (power strips) were not directly connected to a permanently installed branch circuit receptacle. Two power strips were connected and powering two 120-volt Viking Model PA-2A multi-line loud ringer/ paging amplifiers, and a Grandstream network switch.
