29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location: Worksite  On or about June 22, 2023, employees were exposed to falls of over 20 feet while working and walking on the roof and ascending and descending from the extension ladder being utilized to access the roof. No competent person was onsite to inspect the jobsite and equipment, and ensure employee safety. Untrained employees were working at heights of over 20 feet without utilizing any fall protection, the ladder being utilized to access the roof was broken, and an employee was observed carrying equipment in one hand while utilizing the ladder.
