29 CFR 1910.215(d)(1): The employer did not ensure that immediately before mounting, all wheels were closely inspected to make sure they have not been damaged in transit, storage, or otherwise: a) On or about 06/26/23, and at times prior, employees were exposed to a bench mounted circular saw with an abrasive cutting wheel mounted. The wheel was observed to be in an unusable manner. The wheel had bits missing around the periphery and was showing extreme wear and tear. This exposed employees to struck-by hazards.
