29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency was regained: a) Job site, 200 Park Manor Drive, Pittsburgh, PA: On or about June 21, during construction activities, the employer did not ensure that an employee used an aerial lift safely in that he was tied off to the basket and worked outside of it on an adjacent roof, in opposition of the aerial lift manufacturer's directions.
