29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about June 26 and 27, 2023, the employer did not ensure fall protection was implemented for each employee performing commercial roofing operations on a pitched roof. Employees were exposed to the hazard of falling approximately twelve (12) feet.

Employees attached personal fall arrest equipment near the edge of the roof and used excessive rope lengths.
