29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  For Citation 1, Item 1, issued on 12/12/2023 with an abatement date of 8/2/2024, the employer did not submit documents demonstrating complete abatement of the cited violation to OSHA. b)  For Citation 1, Item 2a, issued on 12/12/2023 with an abatement date of 8/2/2024, the employer did not submit documents demonstrating complete abatement of the cited violation to OSHA. c)  For Citation 1, Item 2b, issued on 12/12/2023 with an abatement date of 8/2/2024, the employer did not submit documents demonstrating complete abatement of the cited violation to OSHA.\

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about June 26 and 27, 2023, the employer did not ensure fall protection was implemented for each employee performing commercial roofing operations on a pitched roof.  Employees were exposed to the hazard of falling approximately twelve (12) feet.

29 CFR  1926.502(d)(15):  Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows:  As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:     On or about June 26 and 27, 2023, the employer did not ensure anchorage used for attachment of personal fall arrest equipment was capable of supporting at least 5,000 pounds.  Employees attached personal fall arrest equipment to flue vent pipe and other roof protrusions.

29 CFR  1926.502(d)(16)(iii):  Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   On or about June 26 and 27, 2023, the employer did not ensure that personal fall arrest system fall and stopping distances prohibited a fall of greater than 6 feet and ground contact.  Employees attached personal fall arrest equipment near the edge of the roof and used excessive rope lengths.
