29 CFR 1910.178(m)(1): Powered industrial truck was driven up to an employee who was standing near a fixed object. a) 750 Dowd Ave, Elizabeth, NJ 07201- A forklift was operated while an employee was standing between a printer head and the wall of a semi-trailer. The forklift operator attempted to remove the printer head, which was standing on two 3x6 wooden beams, when it fell forward causing serious injuries to the employee. Incident occurred on 6/21/23.

29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged: a) 750 Dowd Ave, Elizabeth, NJ 07201- A forklift truck was unloading a printer head from a semi-trailer. The printer head was standing and resting on two 3x6 wooden beam. The forklift was trailing the load when the load became unstable and fell forward causing serious injures to an employee who was standing next to the printer head. Incident occurred on 6/21/23.
