29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about June 16, 2023, the employer did not report within 24 hours the in-patient hospitalization injury of an employee who fell approximately 5 feet from a baker scaffold, while performing demolition and asbestos remediation work, at a worksite located at 19600 North Park Boulevard, Shaker Heights, OH 44122. OSHA was not notified of the in-patient hospitalization injury until on or about June 26, 2023 from the employer.
