29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 3b. 29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 27, 2023, employees engaged in shingling activities on a steep slope residential roof were using a pneumatic nailing gun without eye or face protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 26, 2023, workers were observed engaged in residential roofing using portable ladders extended less than 3ft above the upper landing surface to access and egress the roof.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about June 27, 2023, the employer failed to provide each affected employee with ladder safety training. An individual was observed struggling to exit the upper landing surface due to the ladder not being extended 3ft above the door entry portico.
