29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Antigua at Lakewood Ranch - 12581 Cades Bay Circle:  On or about June 27, 2023, the employer exposed employees to fall hazards while installing wooden floor joists on top concrete block walls without the use of  fall protection.    b) Antigua at Lakewood Ranch - 12585 Cades Bay Circle: On or about June 27, 2023, the employer exposed employees to fall hazards while installing wooden floor joists on top concrete block walls without the use of  fall protection.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a) Antigua at Lakewood Ranch - 12585 Cades Bay Circle:   On or about June 27, 2023, the employer exposed employees to fall hazards when using an eight foot Werner A Frame Ladder for access to the top of a wooden framed wall while installing wooden floor joists.

(a)  Antigua at Lakewood Ranch - 12581 Cades Bay Circle:  On or about June 27, 2023, the employer exposed employees to falls while accessing the upper working area, approximately 10 feet above the ground while using a metal 21-foot Louisville Extension Ladder where the base horizontal distance was in excess of one-quarter of its working length.
