29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about June 28, 2023 at the location of 3256 Warren Road, Cleveland, OH: The employer did not ensure that employees performing roofing activities on a low slope roof were protected from fall hazards. Employees were exposed to fall hazards of 20-22 feet to the ground below while performing roof tear off and membrane replacement. At the time of inspection warning line systems were not in place and no safety monitor was available or utilized on the roof.   E & G Construction & Roofing was previously cited for a violation of an equivalent  safety and health standard ,29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1328054, citation number 2, item number 1 and was affirmed as a final order on 8/6/2018, with respect to a workplace located at 71/73 Greencrest Terrace, Akron, Ohio 44313.
