29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about June 26, 2023, an employee was operating a Safen Darley Press Brake (Model - H-Brake 320T Ultra & Serial No. 12.2978), and the employer did not ensure employees fingers and hands were protected from being pinched between the part being bent in the press brake and the top side of the press brake die. This exposed the operator to a crushing and amputation hazard.

29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about June 28, 2023, employees operating the AccurPress (Machine No.1058, Model No. 7608 & Serial No. 5892) in the Laser Fabrication Department were exposed to a crushing and amputation hazard while using the press brake to stamp letters into a part. The employer did not ensure the point of operation was properly guarded, in-that the press brake was being operated with a foot pedal and the employees hands within 2 inches of the point of operation.
