29 CFR 1910.1048(d)(1)(i): Each employer who has a workplace covered by this standard shall monitor employees to determine their exposure to formaldehyde. On or about May 16, 2023, at a facility located at 1650 North Street in Neenah, WI; the employer did not monitor employees to determine their exposure to formaldehyde when employees washed out a truck tanker trailer that had contained a formaldehyde solution.

29 CFR 1910.134(e)(4)(i): The medical questionnaire and examinations shall be administered confidentially during the employee's normal working hours or at a time and place convenient to the employee. The medical questionnaire shall be administered in a manner that ensures that the employee understands its content: At a facility located at 1650 North Street in Neenah, WI; the employer did not administer the respirator clearance medical questionnaire confidentially. When asked to provide medical clearance records for respirator use, the employer provided a copy of an employee's completed medical questionnaire, which the employer should not have maintained.
