29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Top of house - On or about June 27, 2023, the employer instructed his employees to apply roofing shingles to a steep pitched roof without ensuring they are protected from falling with the use of a fall protection system exposing them to fall hazards.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Front and rear side of house - On or about June 27, 2023, the employer had his employees apply roof shingles to the roof using aluminum extension ladders that were not extending 3 feet above the roofs edge exposing employees to fall hazards.
