29 CFR 1910.212(a)(1): Machine Guarding. Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about June 22, 2023, the employer did not ensure the HP4 Assembly Station was adequately guarded in that a pinch point was present while raising the inner hub and aligning the holes with the outer drum. b) On or about June 29, 2023, the employer did not ensure that the HP4 lift table was adequately guarded in that there was a 2" opening between the guard and the moving lift table that created a pinch point. c) On or about June 29, 2023, the HP4 Stud press was not adequately guarded in that the installed guard was open on the back and was approximately 8 inches to the point of operation. d) On or about June 29, 2023, the HP4 Fat Boy press point of operation was not adequately guarded in that there was an opening approximately 1.25 inches high and 7.5 inches to the point of operation.

29 CFR 1910.212(a)(3)(iii):Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. a) On or about June 22, 2023 the employer did not provide special handtools to permit easy handling of wheel hubs assemblies without the operator placing a hand in a pinch point area while assembling wheel hubs at HP4.

29 CFR 1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On or about June 29, 2023 the employer did not report an employee's amputation to OSHA within twenty-four hours. The employee amputation occurred on June 22, 2023, and was not reported to OSHA.
