29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At the worksite located at 3209 Pear St., Saint Joseph MO, employees were exposed to fall hazards from a supported scaffold being lowered from the roof by a forklift without guard rails along the front  to prevent them from falling out when being lowered to the ground from the roof.. This was documented on or about 6-27-2023.

29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  At the worksite located at 3209 Pear St., Saint Joseph MO, employees were exposed to fall hazards from a open skylight being replaced when the employees were not protected by personal fall arrest systems, covers, or guardrail systems erected around such holes. This was documented on or about 6-27-2023.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  At the worksite located at 3209 Pear St., Saint Joseph MO, employees were exposed to fall hazards when conducting waterproofing work on top of a low slope roof with unprotected sides and edges and were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. . This was documented on or about 6-27-2023.
