(a) On or about June 26, 2023, an outside contractor and a host site employee were engaged in servicing and maintenance on the Shanklin Heat Sealer. Employees associated with the maintenance task were not safely positioned or removed from the work area prior to cycling the machine exposing the employee(s) to a caught-in and amputation hazard.
