29 CFR 1915.158(b)(2):On floating vessels under 200 feet in length, at least one 30-inch U.S. Coast Guard approved ring life buoy with line attached were not located at the gangway. Boatyard, dock: The employer did not provide a throw ring near where an employee was working on the Annie Jane boat over water. The life ring was not located on the floating dock, but at the top of the gangway 25 feet away from the boat. There was no line attached.
