29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a) Building Exterior:  Employee were exposed to an 25 foot fall hazard while changing lentils from a tubular welded frame scaffold with missing planks. Scaffold was not fully decked or planked between front and uprights, on or about 6/26/23

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   a) Building Exterior: A proper means of access was not provided for the scaffold system. Thereby exposing employees to a fall hazard, on or about 6/26/23 Employees were using the scaffold frame to access the scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Building Exterior:  Employees were exposed to a 25' fall hazard while changing lentils from a pipe scaffold that was not provided with a guardrail system or other means of fall protection, on or about 6/26/23.

29 CFR 1926.452(b)(3): On straight run scaffolds, longitudinal bracing across the inner and outer rows of posts were not installed diagonally in both directions, and did not extend from the base of the end posts upward to the top of the scaffold at approximately a 45 degree angle. On scaffolds whose length is greater than their height, such bracing were not repeated beginning at least at every fifth post. On scaffolds whose length is less than their height, such bracing were not installed from the base of the end posts upward to the opposite end posts, and then in alternating directions until reaching the top of the scaffold. Bracing were not installed as close as possible to the intersection of the bearer and post or runner and post.   a) Building Exterior:  Employees were working on a scaffold that was missing the inner cross braces along the entire pipe scaffold length, on or about 6/26/23.
