29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) On or about June 28, 2023, located on the shop floor of 123 W. Boxhorn Dr. Mukwonago, WI 53149, employees were able to access the point of operation that was not guarded on the Allsteel industrial 45-ton press brake, manufactured by Ferro Products Co. Ltd., model number 45.8, serial number Y604, exposing employees to crushing hazards and amputation injuries. (b) On or about June 28, 2023, located on the shop floor of 123 W. Boxhorn Dr. Mukwonago, WI 53149, employees were able to access the point of operation that was not guarded on the backside of the Allsteel industrial hydraulic 45-ton press brake, manufactured by Ferro Products Co. Ltd., model #45.8, serial number Y604, exposing employees to crushing hazards and amputation injuries.

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. (a) On or about June 30, 2023 at the location 123 Boxhorn Dr. Mukwonago, WI 53149 in the of fabrication shop, the Accurshear 250 ton press brake had damaged electrical wiring in the foot pedal actuator, exposing employees to electrical hazards.
