Ltd., model number 45.8, serial number Y604, exposing employees to crushing hazards and amputation injuries.  (b) On or about June 28, 2023, located on the shop floor of 123 W. Boxhorn Dr. Mukwonago, WI 53149, employees were able to access the point of operation that was not guarded on the backside of the Allsteel industrial hydraulic 45-ton press brake, manufactured by Ferro Products Co. Ltd., model #45.8, serial number Y604, exposing employees to crushing hazards and amputation injuries.
