(a) On or about June 30, 2023, at the jobsite located at 5100 S. 76th Street Greendale, WI 53129, plywood being used to cover a hole in the deck of the roof was not capable of supporting an employee without failure at least twice the weight of employees. This exposed employees conducting decking and framing work on the roof a new commercial building to an approximately 25 foot fall to the ground.

(a) On or about June 30, 2023, at the jobsite located at 5100 S. 76th Street Greendale, WI 53129, employees conducting decking and framing work on the roof a new commercial building were exposed to an approximately 25 foot fall, due to unsecure plywood hole covers in the roof floor of a commercial building being constructed.
