29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. Location: Worksite On or about July 5, 2023, employees were exposed to fall hazards where the employer's safety training, written safety & health program, new employee orientation with exam, and weekly toolbox talks are written and given in English. All of the training materials are in English. The majority of the workers' primary languages are Spanish and Portuguese.

29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Location: Worksite On or about July 5, 2023, the employer failed to protect employees from falling through skylights located next to the work area up on the roof. No personal fall arrest systems, covers or guardrail systems were erected around the skylights to protect employees, and an employee fell through a skylight, more than 35 feet down to the concrete floor below within the facility.

29 CFR 1926.1053(b)(8):Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. Location: Worksite On or about July 5, 2023, employees were exposed to falls where a barricade system is not in place around the bottom of the extension ladder which is used by all employees to access the roof. The ladder is next to the parking spaces, and nothing is in place to keep vehicles away from possibly knocking the ladder over.
