Location: 17 Green Road, Medford, MA;  On or about 07/05/2023, an employee was not tied off to fall protection and fell 18-feet from the porch roof into the open basement of the residence.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  Location: 17 Green Road, Medford, MA;  On or about 07/05/2023, in lieu of using an anchorage point the lifeline was wrapped around the brick chimney and clipped to itself. There is no documentation to support this configuration being able to support the 5,000lbs of force per employee attached.
