Location: 11 Old Boston Road, Tewksbury, MA; On or about 07/05/2023, one employee was observed not tied off to fall protection while working from a Snorkel aerial lift (model#: TB86J and serial#: TB86J-04-180600421), and another was observed incorrectly tied off to the guardrail of the aerial lift basket with the lanyard secured to itself in a JLG SkyPower aerial lift.
