29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width, by a safety monitor system alone:   a) Job site, 6572 Lincoln Highway, Jeanette, PA: On or about July 5, 2023, during roofing activities, the employer did not ensure that an employee used appropriate fall protection systems/devices where exposed to a fall hazard of up to at least approximately 8 feet, 3 inches.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 6572 Lincoln Highway, Jeanette, PA: On or about July 5, 2023, during roofing activities, the employer did not ensure that an employee had a safe way of getting on/off the roof in that the ladder used to do so extended only about 2 feet above the roof line and there was no grab rail or other similar device provided.
