29 CFR 1910.29(e)(1):29 CFR 1910.29(e)(1): The employer did not ensure that each cover for a hole in a walking-working surface is capable of supporting without failure, at least twice the maximum intended load that may be imposed on the cover at any one time. a) Jobsite: 6401 Martins Mill Rd, Philadelphia, PA 19111, Commons building - On or about June 28, 2023, the employer did not ensure the covers on the skylights could support at least twice the maximum intended load that may be imposed on the covers prior to directing employees to service them. An employee fell on a skylight cover fractured it and fell approximately 12' 9" to the lower level.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment. a) Jobsite: 6401 Martins Mill Rd, Philadelphia, PA 19111, Commons building - On or about June 28, 2023, the employer did not asses the roof of the building for hazards requiring the use of personal protective equipment. An employee servicing a skylight on the roof of the building, without the use of any personal protective equipment, fell through the skylight approximately 12' 9" to the lower level.
