29 CFR 1926.503(a)(2)(iii): The employer shall ensure that each employee has been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On or about July 6, 2023, at a worksite located at 309 East Main Street., Hortonville, Wisconsin: the employer did not ensure that employees utilizing personal fall arrest systems were trained in the correct installation of fall protection systems, including connections to anchor points.
