29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about July 10, 2023 the employer RP Masonry and Roofing LLC. at jobsite located at 3266 NE Catawba Rd., Port Clinton, OH 43452, did not have a plan or a program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as fall hazards during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 10, 2023 the employer RP Masonry and Roofing LLC. at jobsite located at 3266 NE Catawba Rd., Port Clinton, OH 43452, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about July 10, 2023 the employer RP Masonry and Roofing LLC. at jobsite located at 3266 NE Catawba Rd., Port Clinton, OH 43452, did not ensure employees were protected from falls while performing roofing activities atop a steep sloped commercial structure. Two employees performing metal roofing installation on the structure were exposed  to falls of 16 feet or greater from the peak level, and falls of 12 feet or greater from the eave level roof to the ground below. The roof was a 8-12 pitch.
