29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On July 6, 2023, at the above addressed jobsite, employees performing roofing work on the second story of a new construction residential home were exposed to fall hazards of approximately 20 feet.

29 CFR  1926.1053(b)(1): Where portable ladder(s) were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) On July 6, 2023, at the above addressed jobsite, the portable ladder used by employees to access and egress the roof, measuring approximately 20 feet from ground to eve, was not extended to have the side rails extend at least three (3) feet above the roofline.
