29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about June 29, 2023: Employees engaged in cleaning the box loader at the Nebraska Plastics factory located at 700 W. Hwy 30, Cozad, Nebraska, were exposed to amputation hazards in that the employer had not developed and utilized energy control procedures for the machine. An employee received a fingertip amputation injury while cleaning the box loader because its air lock was energized and rotating.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 psi: Employees engaged in plastic production activities at the Nebraska Plastics Factory located at 700 W. Hwy 30, Cozad, Nebraska were exposed to struck by hazards in that they used high pressure air to clean paint residue and it had not been reduced to 30 PSI or less.
