29 CFR  1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about June 29, 2023: Employees engaged in cleaning the box loader at the Nebraska Plastics factory located at 700 W. Hwy 30, Cozad, Nebraska, were exposed to amputation hazards in that the employer had not developed and utilized energy control procedures for the machine.  An employee received a fingertip amputation injury while cleaning the box loader because its air lock was energized and rotating.
