29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: On or about July 5, 2023, and at times prior thereto, workers used a Star Industries attachment (trash hopper) with a Genie GTH-1056 Telehandler that was not approved by the manufacturer (Genie). Exposing the workers to struck-by and tipping hazards.
