29 CFR 1910.22(a)(1):  The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  (a) On or about July 6th, 2023, the employer did not ensure that employees were protected from injury in that wood scraps and saw dust were not effectively cleaned.  This condition exposed employees to injuries associated with slips, trips and falls.  (b) On or about July 6th, 2023, the employer did not ensure that employees were protected from injury in that flexible cords, tools, and manufacturing materials were stored in a cluttered and unsafe manner throughout the facility.  This condition exposed employees to injuries associated with slips, trips and falls.  (c) Wood Shop, Router Station:  On or about July 6th, 2023, and at times prior, employees were exposed to fire and explosion hazards in that, combustible materials with a maximum normalized DP/DT of 10.46 and a moisture content of 6.6 in the Wood Shop, Router Station.  The sampled material was determined to be ignitable and combustible.  This condition exposed employees to fire hazards.  (d) Wood Shop, DeWalt Saw Station, adjacent to Route Station:  On or about July 6th, 2023, and at times prior, employees were exposed to fire and explosion hazards in that, combustible materials with a maximum normalized DP/DT of 13.71 and a moisture content of 6.3 in the Wood Shop, DeWalt Saw Station.  The sampled material was determined to be ignitable and combustible.  This condition exposed employees to fire hazards.  (e) Wood Shop, DeWalt Saw Station, adjacent to electrical panels:  On or about July 6th, 2023, and at times prior, employees were exposed to fire and explosion hazards in that, combustible materials with a maximum normalized DP/DT of 13.54 and a moisture content of 6.2 in the Wood Shop, DeWalt Saw Station.  The sampled material was determined to be ignitable and combustible.  This condition exposed employees to fire hazards.  (f) Wood Shop, Bandsaw Station (Roller 265):  On or about July 6th, 2023, and at times prior, employees were exposed to fire and explosion hazards in that, combustible materials with a maximum normalized DP/DT of 16.25 and a moisture content of 4.6 in the Wood Shop, Bandsaw Station (Roller 265).  The sampled material was determined to be ignitable and combustible.  This condition exposed employees to fire hazards.  (g) Wood Shop, Baker Machine (blue):  On or about July 6th, 2023, and at times prior, employees were exposed to fire and explosion hazards in that, combustible materials with a maximum normalized DP/DT of 18.61  and a moisture content of 6.5 in the Wood Shop, Baker Machine (blue).  The sampled material was determined to be ignitable and combustible.  This condition exposed employees to fire hazards.

The employer did not ensure that each container of hazardous chemicals in the workplace was labeled with the product identifier and words, pictures, symbols, or combination thereof, which provides at least general information regarding the hazards of the chemicals.  b) On or about July 6, 2023,  employees were exposed to chemical hazards while working with chemicals such as but not limited, to; Liquid Propane Gas, Cyanoacrylate Accelerator, E-Z Bond Instant Glue - Medium (Cyanoacrylate). The employer did not provide the required safety data sheet for each hazardous chemical in use.     c) On or about July 6, 2023, employees were exposed to chemical hazards while working with chemicals such as but not limited, to; Liquid Propane Gas, Cyanoacrylate Accelerator, E-Z Bond Instant Glue - Medium (Cyanoacrylate). The employer did not provide effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.

29 CFR  1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) On or about July 6, 2023, employees exposed to respiratory hazards from not being provided Appendix D which informs the wearer about proper care and maintenance.     Employees were not provided the information listed in Appendix D. Employees are not required to wear any type of respiratory protection except on a voluntary basis. The types of respirators being worn voluntarily are 3M particulate dust masks.
