29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about June 21, 2023, employees installing rebar walls were exposed to fall hazards of up to 20 feet and the employer did not ensure the guardrail system being used was continuous along the entire edge of the walking/working surface.

29 CFR 1926.502(b)(2)(i): Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used between posts, were more than 19 inches (.5 m) apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches (48 cm) wide: On or about June 21, 2023, employees installing rebar walls were exposed to fall hazards of up to 20 feet and the employer did not ensure the guardrail system being used was equipped with a midrail near the northern portion of the east-facing edge of the work area.
