29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  Worksite - Roof:  On or about July 7, 2023, employees were not protected from falling while working on the roof, approximately 18 feet above the ground.  (b) Worksite - Third Floor North Wall:  On or about July 13, 2023, employees were not protected from falling while working from a platform, approximately 16-17 feet above the ground.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:  (a) Ground Floor to Second Floor at Worksite: On or about July 7, 2023, employees accessing and exiting from the 1st to the 2nd floor to perform work, were exposed to fall hazards while climbing this stairway, with six (6) stairs, which was not equipped with a stair rail system on the unprotected side.
