29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded: a) New residential construction located at Lot #39 on Dee Kennedy Rd., Hoschton, GA 30548, front: On or about 07/07/2023, the employer exposed employee to severe laceration/ amputation hazards while operating a trailer mortar mixer, in that the starter recoil cover was missing which exposed employees to moving unguarded parts.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) New residential construction located at Lot #39 on Dee Kennedy Rd., Hoschton, GA 30548, Scaffold: On or about 07/07/2023, and at times prior, the employer exposed employees to fall hazards of approximately 19-feet, 5-inches while working on a fabricated scaffold system, in that a ladder or other safe means of access was not provided as employees climbed scaffold frames and cross-bracing to access each level.
