29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about June 28, 2023, and at times prior, at 160 Aviation DR N Naples Florida, an employee who has not received training in the recognition and avoidance of unsafe conditions related to rigging operation was exposed to struck by, crush by and amputation hazard while placing his left hand between the alloy steel chain and the concrete pipe being tightened.
