29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 8779 Hugo Dive, Littleton CO 80125: On or about June 23, 2023, and at times prior, the exposing employer did not ensure that employees were protected from falls of 19.74 feet where employees were cutting and installing plywood sheeting on the second story without the use of fall protection.  Additionally, employees were exposed to falls from unprotected edges on the second floor where no fall protection such as guardrails were installed, and openings would allow for a fall to a lower level more than 6 feet below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  8779 Hugo Dive, Littleton CO 80125: On or about June 23, 2023, and at times prior, the exposing employer did not ensure that the extension ladder used to access the second floor was extended at least 3 feet above the upper landing surface, exposing employees to fall hazards.
