29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  1175 Bay Rd. Amherst, MA 01002: On or about July 7, 2023, the employer did not ensure that employees were using eye protection while using pneumatic nail guns during shingle installation on a residential rooftop.

Amherst, MA 01002: On or about July 7, 2023, two employees were exposed to fall hazards of approximately 9 feet 5 inches above a lower level while performing residential construction work and were not protected from falling by guard rail systems, safety net systems or a personal fall arrest system.

Amherst, MA 01002: On or about July 7, 2023, the employer had not trained employees to recognize fall hazards and on the procedures to be followed to minimize these hazards, prior to exposing them to fall heights of approximately 9 feet 5 inches during a residential construction project.

Amherst, MA 01002: On or about July 7, 2023, the employer had not prepared a written certification record of fall protection training record which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for his three employees who were installing shingles on a residential roof and exposed to fall hazards of 9 feet 5 inches or greater.
