29 CFR  1910.132(a):Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about July 11, 2022  at an establishment located at 1411 Eastern Avenue, Plymouth, Wisconsin: employees unpacking frozen items were not provided appropriate hand and eye protection for working with dry ice.

29 CFR  1910.132(d)(1):The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  On or about July 11, 2022  at an establishment located at 1411 Eastern Avenue, Plymouth, Wisconsin: Employer had not conducted an assessment of the workplace to determine if hazards are present, or are likely to be present which necessitate the use of personal protective equipment. Employees are exposed to eye and skin contact with handling dry ice.

29 CFR  1910.303(g)(1)(i)(A):The depth of the working space in the direction of access to live parts may not be less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed;  On or about July 11, 2022  at an establishment located at 1411 Eastern Avenue, Plymouth, Wisconsin: the employer failed to keep clear an unobstructed path of at least 762 mm (30 inches) in front of an electrical disconnects.   a) Shipping and receiving electrical disconnect was blocked with material and equipment.  b) Dairy/Frozen department electrical panel boxes were blocked with materials.  c) Liquor storage area electrical panels were blocked with materials .
