29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems. a. VFP Fire Systems located at 4028 Charter Oak Way, Columbus, OH 43219: On or about July 11, 2023, the employer did not ensure that daily inspections of excavations, the adjacent areas, and protective systems were made by a competent person. An employee working in an excavation at depths of over seven feet without any protective system in place to prevent collapse. Inspections were not performed by a competent person to determine protective measures needed to work in the excavation with these conditions. The following condition exposed employees to struck-by and caught-in hazards.

29 CFR 1926.652(a)(1):Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a. VFP Fire Systems located at 4028 Charter Oak Way, Columbus, OH 43219: On or about July 11, 2023, an employee was working in an excavation of over seven feet in depth on a fire/water main line without any means of cave-in protection in place. The following condition exposed the employee to struck-by/caught-in hazards.
